Virtual Care with a Personal Touch
Our innovative Facilitated Telemedicine program reduces barriers to accessing in-clinic appointments and ensures you receive the high-quality care you deserve from the comfort of your home. A Medical Assistant will come to your home with specialized equipment and connect you virtually with your provider.
In-home services include:
- Blood pressure and vitals
- Lab collection
- Vaccines
- Geriatric assessments
- Perinatal visits
- Medical device teaching
These visits benefit Mary’s Center participants who face difficulties coming onsite for their medical visits due to complex medical, behavioral, or social needs.
NOTE: You must have health insurance to participate in this program. Services are available in Washington, DC, Montgomery County, and Prince George’s County, MD. Facilitated Telemedicine visits do not replace the need to come into the clinic. There may be instances where you will need to come into the clinic in person. Together, you and your clinician can determine a care plan that makes sense for your healthcare needs.

Care Coordination Services
Mary’s Center offers care coordination services through My Health GPS and the Health Partners/Socios de Salud programs. Launched by the DC Department of Health Care Finance, My Health GPS is available to qualified DC Medicaid participants with 3 or more chronic conditions. The Health Partners/Socios de Salud program is available to all participants receiving primary care at Mary’s Center with diabetes, hypertension, or gestational diabetes. These programs support participants in effectively navigating their care and self-managing their conditions, improving health outcomes, and reducing preventable hospital admissions and emergency room visits.
