Are you looking for high-quality healthcare, mental health services, social services and a way to get connected to the community resources you need? The Mary’s Center Health Homes team is here to help you, through our office located at 4302 Georgia Avenue NW, Washington DC 20011..

Mary’s Center became a certified DC Health Home in January 2016, when the Department of Health Care Finance (DHCF) launched the My DC Health Homes program to benefit Medicaid recipients. The program supports the integration of physical, mental, social, and community services. All Health Homes services are optional and will not affect your eligibility for other Medicaid services. All services are covered under Medicaid with no additional fees.

Once you enroll in this program, Health Homes staff begins coordinating all your healthcare needs to help you get and stay healthy.


What to expect when you enroll in Health Homes

 You will be working directly with a Care Coordinator and a Nurse Care Manager who will:

·         Make sure that you get the medical and mental services that you need including help with transportation, appointments reminders, and scheduling as needed.  

·         Consult with Health Homes medical doctor and psychiatrist to ensure that your health care needs are met.

·         Help to ensure that you have all the medications that your healthcare providers have ordered and that you are taking all the medications correctly.

·         Follow up after you are discharge from the hospital

·         Assist you with other resources in the community that can help you and your caregivers improve your health and well-being such as housing, food, transportation and legal assistance.

·         Help you gain access to employment and educational training.

·         Provide support to you and your family in your pathway to better health.
