The transition from adolescence to adulthood can be both exciting and overwhelming, so it’s important to know you’re not alone. Using the “Got Transition” program as a guide, we’ve put together helpful resources to help you navigate this transition and continue caring for your emotional health.

We accept most insurance plans including Medicaid and Medicare. If you don’t have health insurance, we’ll help you apply for public benefits or provide care at a discount. If needed, our bilingual health access program (BHAP) team can help you learn about your eligibility for health insurance and other public benefits.
